My current workflow is to
1) Annotate a pdf,
2) Export for Print to obtain a pdf of the book with margin notes either for selected pages or all pages,
3) Use "Export Outline to Evernote",
4) Copy the text outline generated in Step 3 to the note generated in Step 2,
5) Delete the note generated in Step 3.

Ps few of those steps could be eliminated if "Export Outline to Evernote" could export either or both
1) a pdf of the book with margin notes -- with highlighted pages or all pages, or
2) pdf of mindmap
